This is a 40 year-old patient who had previously had breast implant/lift surgery done elsewhere 9 years ago which consisted of smooth round silicone gel implants placed under the muscle thru the nipple incision in conjunction with a Binelli / periareolar breast lift (incisions around the areola). She presented to my practice with unhappiness in shape and feel. On examination, she had capsular contracture on both sides but more so on the right side. She also complained of burning pain on the right which had been bothering her (symptoms of capsular contracture) and desire for shape improvement. She has unnatural upper pole fullness and tightness (high riding implant due to capsular contracture) and capsular contracture especially on the right side. She wanted to be a little fuller, have a softer feel, as well as a better shape. She underwent removal of old intact implants (style 15 371 Allergan implants) with subtotal capsulectomy, replacement of new more cohesive smooth round silicone gel Inspira Allergan implants… 450 high profile soft-touch more cohesive silicone gel implants with use of Galaflex (acellular dermal matrix) through an inframammary incision (under the breast). Six week after results are shown with softer and more natural appearance, burning discomfort completely gone, as well as a better shape and slope in the upper pole of the breasts.
