Why Would Someone Need Breast Implant Revision Surgery?

Woman Holding Breast in Sports BraBreast augmentation continues to be one of the most popular cosmetic procedures in the United States, offering women the ability to enhance their breast size, improve shape, and restore volume. However, breast implants are not designed to last a lifetime. Over the years, changes in the body, implant wear, dissatisfaction with previous results, or other concerns can make breast implant revision surgery necessary.

Board-certified Houston plastic surgeon James F. Boynton, MD has successfully performed revision breast augmentation for many years, helping women resolve complications that may have arisen, restore balance, and achieve results that look and feel more natural.

Here are some of the reasons women may consider breast implant revision surgery:

1. Implants Have Reached the End of Their Lifespan

Breast implants, whether saline or silicone, are not permanent devices. On average, they may last 10 to 20 years, but some need to be replaced sooner due to rupture, leaks, or changes in shape. Even when implants remain intact, many women eventually decide to exchange them for newer options that offer improved materials, a more natural look, enhanced durability, or a different size.

If an individual has implants of any kind for 10 years or more it is always advisable to make an appointment with a plastic surgeon for an updated examination. Ultrasound imaging can be a very helpful tool to help establish whether implants are still intact or if there are any concerns with rupture. Dr. Boynton often finds that, even in cases of silicone gel rupture, early ruptures are much easier to address, so that is a good reason to follow up with a plastic surgeon’s opinion after 10 years

2. Capsular Contracture

One of the most common reasons women seek revision surgery is capsular contracture, a condition where scar tissue around the implant becomes tight and firm. This can make the breast feel hard, distort its shape, and even cause discomfort. Modern techniques such as capsulectomy (removing the scar capsule) and placement of new implants—sometimes with added tissue support—can restore a softer, more natural look and feel.

3. Implant Displacement or “Shifting”

Over time, implants can move out of their intended position. They may sit too high on the chest (“high-riding”), drop below the breast crease (“bottoming out”), or shift too far to the side. In other cases, implants can drift too close together in the middle of the chest, a condition called symmastia. Revision surgery allows Dr. Boynton to correct the pocket, reposition or exchange implants, and use advanced support materials to help keep them in place.

4. Desire to Change Implant Size or Type

It’s not uncommon for women to reconsider the size or type of implants they originally chose. Some decide to go smaller for a more natural look, while others want a fuller appearance. Additionally, many patients with older saline implants choose to upgrade to modern silicone gel implants, which often feel softer and more natural. Revision surgery makes these changes possible while addressing any issues with the existing implant pocket.

5. Visible Rippling or Wrinkling

In women with thinner breast tissue, implants can sometimes create visible rippling or wrinkling under the skin. Revision techniques may include switching implant types, adjusting placement, or even combining the procedure with fat transfer to add soft tissue coverage. Dr. Boynton often utilizes soft tissue reinforcement scaffolds such as GalaFLEX® to support weakened tissue and improve long-term results.

6. Implant Rupture or Leakage

While implant rupture is relatively rare with modern devices, it can still occur. Saline ruptures are typically obvious, as the implant deflates quickly. Silicone ruptures, on the other hand, may be “silent” and only detected through imaging. Revision surgery can ensure the ruptured implant and surrounding capsule are removed or replaced.

7. Breast Asymmetry or Dissatisfaction with Results

Some women seek revision because they feel their initial augmentation left them with uneven or unnatural-looking results. Breast asymmetry can develop over time as implants shift or as the body changes with weight fluctuations, pregnancy, or aging. Breast revision surgery is customized to address these concerns, often involving repositioning implants, resizing, or combining augmentation with a breast lift.

8. Changes in Natural Breast Tissue Over Time

Even when implants remain intact, natural breast tissue continues to change with age, pregnancy, breastfeeding, weight fluctuations, or hormonal shifts. These changes can lead to sagging, volume loss, or separation of breast tissue from the implant. In some cases, a breast lift performed at the same time as implant revision can restore a youthful, balanced shape.

9. Implant Removal Without Replacement

While many women choose to replace their implants, others decide to remove them altogether. Some prefer to return to their natural breast size, while others may have health concerns or simply no longer feel implants suit their lifestyle. Dr. Boynton offers advanced techniques for breast implant removal (explantation), which can sometimes be combined with a breast lift to refine shape and contour.

Why Choosing a Revision Specialist Matters

Breast implant revision surgery is often more complex than a first-time breast augmentation. Scar tissue, stretched skin, or implant pocket issues can make the procedure more challenging. That’s why selecting a surgeon with specialized expertise in revision breast surgery is so important.
Dr. Boynton has devoted a significant portion of his practice to revision breast augmentation. His experience with techniques such as the neo pocket method, acellular dermal matrices (ADM), GalaFLEX® “scaffolds,” and other advanced approaches allow him to tailor each procedure to the patient’s unique needs. His goal is always the same: to help women achieve natural-looking, comfortable, and lasting results.
