This is a 47 year old patient that had a history of saline subpectoral breast implants via the armpit with a “Binelli” or periareolar breast lift (incision only around the areola) at the same time approximately 13 years ago done elsewhere. She was bothered by the widened diameter of her areola. She also wanted to be smaller, more uplifted, and wanted her “enlarged” diameter areolae to be smaller. As a side note, it is important for me to point out that I have found in 15 years of practice that Binelli breast lifts with implants (as in this case) often can leave an “exploded” or greatly enlarged areolar diameter because of the stretch imparted by the implant. This can be a very challenging problem to fix in aesthetic breast revision surgery. I performed a removal of the old indwelling breast implants with a revision full “inverted T” superior/central pedical breast lift (mastopexy). Her six month after results are shown with improved shape, areolar diameter, and uplift as well as smaller size. The fading “redness” of her incisions are well on their way; however, it typically takes a full 12-14 months to completely fade.
